Q: How do I request a referral or prior authorization?

A: Please have your referring provider fax referral documentation to (301) 603-2294.

Q: How do I access my medical records?

A: Request records through the Privia patient portal at www.myprivia.com/account-access or contact our office.

Q: What should I bring to my first appointment?

A: Photo ID, insurance card, list of current medications (with dosages), relevant prior medical records or imaging, and completed new patient forms.

Q: Do I need a referral?
